CHECKLIST FOR PETS ARRIVING FROM
BRITISH ISLES, AUSTRALIA, GUAM AND NEW ZEALAND
All steps need to be completed in order to qualify for exemption.
1 I understand that my pet(s) must be transported in the same aircraft from the port of origin to
Honolulu International Airport. None of my pets are off-loaded en route to Hawaii from the country
of origin, except that off-loading may be permitted to transfer the animal to another aircraft under
the following conditions only:
A) The off-loading, transfer and placement on the next aircraft must be directly supervised by
the officer in charge of either aircraft;
B) The officer in charge shall certify in writing that the animal in transit did not come in physical
contact with any other animal;
C) The officer in charge shall be identified on the certification by name, position, title, airline
name, business address and phone number; and;
D) The certification shall state the exact date, time, aircraft and flight number for both the
arriving and departing aircraft.
1 When arriving in Hawaii, my pet must be accompanied with the following documents:
A) Health certificate within 14 days of transport certifying free of external parasites and signs of
transmissible disease; by a veterinarian holding an appointment as an officer of the
government of the country of origin.
B) Affidavit executed by owner or consignor certifying the animal was not outside the Country
of origin during the six-month period immediately prior to date of shipment.
C) Affidavit executed by the captain of the aircraft or senior station official attesting that the
animal was transported on one aircraft.
1 If my pet is originating from Guam, I have a properly filled out and signed Guam Affidavit, form IC-
5, to provide to State inspectors upon my pet’s arrival in Hawaii.
1 My pet will be shipped in containers that are sealed such that removal of the animal will break the
seal.
1 I understand that all animals shall be inspected by station veterinarian or inspector at time of
arrival and be subject to treatment for external parasites. Any indication of transmissible disease
in any animals in the shipment shall be sufficient reason to quarantine all of the animals at the
quarantine station.
1 I have arranged for my pet to arrive at the Airport Animal Quarantine Holding Facility at Honolulu
International Airport during normal inspection hours of 8:00 AM to 4:30 PM. I realize that it may
usually take up to one hour for the airlines to transport my pet to the Airport Animal Quarantine
Holding Facility [Phone: (808) 837-8092] and qualifying animals not arriving at the facility during
normal inspection hours will be released the following day. Posted
inspection hours are subject to
change.

I understand that animals on international flights will also need to clear U.S. Customs before they
may be released from the Airport Animal Quarantine Holding Facility.

1 Due to Federal Security regulations at the Honolulu International Airport, all pets must be secured
in their flight carriers when picked up from the Airport Animal Quarantine Holding Facility until the
animal is out of the Airport.
1 I understand that release at Honolulu International Airport will not be possible when prohibited by
Federal airport security advisories. Under those circumstances pets will be released the following
day at the Animal Quarantine Station 99-951 Halawa Valley Street, Aiea, Hawaii 96701, phone:

Monday through Friday: 9:00 AM to 11:00 AM & 1:00 PM to 4:30PM
Saturday, Sunday, State Holidays: 8:00 AM to 10:00 AM & 12:00 PM to 3:30PM
